Fix a bug where a GPUI macro still used `ModelContext`
Rename `AsyncAppContext` -> `AsyncApp`
Rename update_model, read_model, insert_model, and reserve_model to
update_entity, read_entity, insert_entity, and reserve_entity

There's still a bit more work to do on this, but this PR is compiling
(with warnings) after eliminating the key types. When the tasks below
are complete, this will be the new narrative for GPUI:
- `Entity<T>` - This replaces `View<T>`/`Model<T>`. It represents a unit
of state, and if `T` implements `Render`, then `Entity<T>` implements
`Element`.
- `&mut App` This replaces `AppContext` and represents the app.
- `&mut Context<T>` This replaces `ModelContext` and derefs to `App`. It
is provided by the framework when updating an entity.
- `&mut Window` Broken out of `&mut WindowContext` which no longer
exists. Every method that once took `&mut WindowContext` now takes `&mut
Window, &mut App` and every method that took `&mut ViewContext<T>` now
takes `&mut Window, &mut Context<T>`
Not pictured here are the two other failed attempts. It's been quite a
month!

This PR introduces a new entity called Project Tree which is responsible
for finding subprojects within a worktree;
a subproject is a language-specific subset of a worktree which should be
accurately tracked on the language server side. We'll have an ability to
set multiple disjoint workspaceFolders on language server side OR spawn
multiple instances of a single language server (which will be the case
with e.g. Python language servers, as they need to interact with
multiple disjoint virtual environments).
Project Tree assumes that projects of the same LspAdapter kind cannot
overlap. Additionally project nesting is not allowed within the scope of
a single LspAdapter.

When debugging git diffs we found that this introduced a re-ordering of
messages sent to the LSP:
* User hits "format"
* Zed adjusts spacing, and sends "spaces changed" to the LSP
* Zed sends "format" to LSP
With the async approach here, the format request can now arrive before
the space changed request.
You can reproduce this with `test_strip_whitespace_and_format_via_lsp`
under some conditions.

This is a follow-up to #12640.
While profiling latency of working with a project with 8192 diagnostics
I've noticed that while we're parsing the LSP messages into a generic
message struct on a background thread, we can still block the main
thread as the conversion between that generic message struct and the
actual LSP message (for use by callback) is still happening on the main
thread.
This PR significantly constrains what a message callback can use, so
that it can be executed on any thread; we also send off message
conversion to the background thread. In practice new callback
constraints were already satisfied by all call sites, so no code outside
of the lsp crate had to be adjusted.
This has improved throughput of my 8192-benchmark from 40s to send out
all diagnostics after saving to ~20s. Now main thread is spending most
of the time updating our diagnostics sets, which can probably be
improved too.

I've noticed that I cannot work with any Zig projects, as there were no
"go to definition", formatting and inlay hints.
After debugging, I've discovered that `typos` was registered as a first
language server, becoming the "primary" one for Zig.
That one does not have any proper capabilities, hence all corresponding
LSP requests were no-op.
While this solution is not ideal (I wonder, how many other set-ups are
broken due to the same thing?), we'd better fix things for now this way
at least.

Notably, RA will now rename module references if you change the source
file name via our project panel.
This PR is a tad bigger than necessary as I torn out the Model<> from
didSave watchers (I tried to reuse that code for the same purpose).

This fixes the issue of multiple language servers showing up as `node`
in the language server logs dropdown.
It does this by changing `language_server.name()` to return the
adapter's name, not the binary name, and changing types to make sure

As Zed instantly shows completion items in the completion menu, and the
resolve will cause the details to appear, flickering.
We can safely resolve the `documentation`, `additionalTextEdits` and
`command` fields, the rest should be resolved eagerly for now.

for all language servers including those defined by extensions.

This fixes https://github.com/zed-industries/zed/issues/13633 by not
sending `source.organizeImports` to the ESLint language server anymore.
Turns out that ESLint tells us through its capabilities that it doesn't
support that code action kind, but we ignored that.
What this code does is to check whether a given server supports specific
code action kinds.
It does this in two places:
1. When constructing the request: we now filter down the list of
requested `kinds`, in case we can do so. If we can't filter down the
list, we keep the previous behavior of sending the
`language_server.code_action_kinds()`
2. Before sending the request: we now check whether the server even
supports sending the request.
This fixes the issue by only sending actions to servers that support it.
I tested this with various language servers and setups and everything
still works (or works better). But of course there are a ton of
different combinations of language servers and code actions and file
types, so I couldn't test them all.

`source.organizeImports` code action was used on save. Zed now filters
out code actions sent to the language servers by checking whether they
are supported first.

Zed currently does not support pull diagnostics, yet still has the
capability for it (`textDocument/diagnostic`) (added in
14993e0876).
Some language servers therefore assume Zed will use pull diagnostics,
which leads to there being no diagnostics at all. This PR removes this
capability, making it possible to get diagnostics with more language
servers.

Due to lifetime extension rules, we were holding onto the request
handler map mutex during parsing of the request itself. This had no
grand repercussions; it only prevented registering a handler for next
request until parsing of the previous one was done.
