Fix bug in following

Prior to this change you could only follow across workspaces when you
were heading to the first window.

Ensure notifications are dismissed
...
Before this change if you joined a project without clicking on the
notification it would never disappear.
Fix a related bug where if you have more than one monitor, the
notification was only dismissed from one of them.

Re-emit notifications and events from ActiveCall
...
This lets us only observe and subscribe to the active call without
needing to track the underlying `Room` if it changes, which implies
writing the same boilerplate over and over.
