Adds a new `documentation` method to actions, that is extracted from doc
comments when using the `actions!` or derive macros.
Additionally, this PR adds doc comments to as many action definitions in
Zed as possible.

In #32656 I generalized the argument to change selections to allow
controling both the scroll and the nav history (and the completion
trigger).
To avoid conflicting with ongoing debugger cherry-picks I left the
argument as an `impl Into<>`, but I think it's clearer to make callers
specify what they want here.
I converted a lot of `None` arguments to `SelectionEffects::no_scroll()`
to be exactly compatible; but I think many people used none as an "i
don't care" value in which case Default::default() might be more
appropraite

Instead of a menagerie of macros for implementing `Action`, now there
are just two:
* `actions!(editor, [MoveLeft, MoveRight])`
* `#[derive(..., Action)]` with `#[action(namespace = editor)]`
In both contexts, `///` doc comments can be provided and will be used in
`JsonSchema`.
In both contexts, parameters can provided in `#[action(...)]`:
- `namespace = some_namespace` sets the namespace. In Zed this is
required.
- `name = "ActionName"` overrides the action's name. This must not
contain "::".
- `no_json` causes the `build` method to always error and
`action_json_schema` to return `None`
and allows actions not implement `serde::Serialize` and
`schemars::JsonSchema`.
- `no_register` skips registering the action. This is useful for
implementing the `Action` trait
while not supporting invocation by name or JSON deserialization.
- `deprecated_aliases = ["editor::SomeAction"]` specifies deprecated old
names for the action.
These action names should *not* correspond to any actions that are
registered. These old names
can then still be used to refer to invoke this action. In Zed, the
keymap JSON schema will
accept these old names and provide warnings.
- `deprecated = "Message about why this action is deprecation"`
specifies a deprecation message.
In Zed, the keymap JSON schema will cause this to be displayed as a
warning. This is a new feature.
Also makes the following changes since this seems like a good time to
make breaking changes:
* In `zed.rs` tests adds a test with an explicit list of namespaces. The
rationale for this is that there is otherwise no checking of `namespace
= ...` attributes.
* `Action::debug_name` renamed to `name_for_type`, since its only
difference with `name` was that it
* `Action::name` now returns `&'static str` instead of `&str` to match
the return of `name_for_type`. This makes the action trait more limited,
but the code was already assuming that `name_for_type` is the same as
`name`, and it requires `&'static`. So really this just makes the trait
harder to misuse.
* Various action reflection methods now use `&'static str` instead of
`SharedString`.

This PR also introduces `Context::processor`, a sibling of
`Context::listener` that takes a strong pointer to entity and allows for
a return result.

Removing the `track_focus` method fix the focus hijack that we
introduced in the previous PR. cc @0xtimsb
Also, I don't see any immediate affect in the actual scrollbar behavior
without this! Holler if that's not the case.

Now, you can pass `show_scrollbar` to Picker that implement a
`uniform_list`. If that's on, the scrollbar should auto-hide if you move

## Context
While looking through the client crate, I noticed that some of the old
functions and variables were still using gpui::model name that was
deprecated during the gpui3 transition. This PR renames those instances
of model to entity to be more inline with gpui3.
In addition, I also renamed `model` to `entity` in cases found by the
below search terms given by @someone13574
- model = cx.
- model: Entity
- model: &Entity
- OpenedModelHandle
- model.update
- model.upgrade
- model = .*\.root (regex)
- parent_model
- model = cx.new
- cx.spawn(move |model

Previously, editor elements had to listen for mouse_up events to
determine when a click had completed. This meant that they only had
access to modifier keys that were pressed during the mouse_up event.
This led to some incorrect user experiences, such as executing a
ctrl+click if the user pressed ctrl after pressing the mouse button, but
before releasing it.
This change adds a click event handler to EditorElement, and adds a
modifier() method to the ClickEvent, which only includes the modifier
keys that were pressed during both mouse down and mouse up. The code for
handling link clicks has been moved into the click event handler, so
that it's only triggered when the non-multi-cursor modifier was held for
both the mouse down and mouse up events.
Closes#12752, #16074, #17892 (the latter two seem to be duplicates of
the former!)

down but before mouse up still triggered ctrl/cmd+click behavior (e.g.
"go to definition")
There's still a bit more work to do on this, but this PR is compiling
(with warnings) after eliminating the key types. When the tasks below
are complete, this will be the new narrative for GPUI:
- `Entity<T>` - This replaces `View<T>`/`Model<T>`. It represents a unit
of state, and if `T` implements `Render`, then `Entity<T>` implements
`Element`.
- `&mut App` This replaces `AppContext` and represents the app.
- `&mut Context<T>` This replaces `ModelContext` and derefs to `App`. It
is provided by the framework when updating an entity.
- `&mut Window` Broken out of `&mut WindowContext` which no longer
exists. Every method that once took `&mut WindowContext` now takes `&mut
Window, &mut App` and every method that took `&mut ViewContext<T>` now
takes `&mut Window, &mut Context<T>`
Not pictured here are the two other failed attempts. It's been quite a
month!

Zed does not scroll always, but only if the item is out of sight, this
is preserved for now.
Otherwise, if the item is out of sight, project and outline panels + the
syntax tree view now attempt to scroll it into the middle, if there's
enough elements above and below.

Using the file system as a database seems like it's easy, but it's
actually a real pain. I'd like to use LMDB to store the prompts locally
so we have more control. We can always add an export option, but I want
the source of truth to be somewhere other than the file system.
So far, I have a PromptStore which is global to the application and can
be initialized on startup. Then there's a `PromptLibrary` which is
intended to be the root of a new kind of Zed window. I haven't actually
seen pixels yet, but I've sketched out the basics needed to create a new
prompt, save, etc.
Still lots to figure out but the foundations of being backed by a DB and
rendering in an independent window are in place.

Adds the ability to have some effect run when a selection changes in a
picker.
If the `PickerDelegate` implements something other than `None` for
`selected_index_changed` then each time the selection changes it will
run that effect.
For example:

the functionality we intended for the prompt library, we're changing

telescope.
